Welcome to Neurosurgeons for Children. We hope this information will be helpful in answering some of your questions about our procedures and policies.
At the time of your first visit, you will be required to complete several forms. Each of those forms is available below if you prefer to complete them prior to your arrival.

Insurance policies
All services performed are the financial obligation of the patient or named responsible party. Fees for office visits are expected to be paid at the time services are rendered. (However, we will collect only your co-pay or co-insurance if you provide the appropriate documentation of insurance coverage the time of your visit.) If a surgical procedure is required, you may contact the business office at (214) 456-6654 to discuss payment arrangements prior to the surgery.
As a courtesy to you, Neurosurgeons for Children agrees to file your insurance claims based on the information provided by the responsible party. It is imperative that you provide complete and current insurance information so we can continue to offer this service. We encourage you, as a policyholder, to contact your carrier and to monitor the status of your claim.
Our goal is to provide the best care and services for our patients. To assist us in achieving our goal, we depend on you, our patient, to let us know how we are doing. Please contact our business office if you have any questions about our billing policies or wish to make special arrangements. Medicare
We accept assignment for patients with Part B Medicare coverage. Medicare patients will be expected to pay applicable deductible and co-pay amounts.
Medicaid/CSHCN
We are Texas Medicaid and CSHCN providers; however, Neurosurgeons for Children does not participate with Texas Health Network, Parkland HEALTHfirst or Americaid under the STAR program.
The STAR program has been implemented in Dallas, Rockwall, Collin, Navarro, Kaufman, Ellis and Hunt counties. If you currently receive Medicaid benefits under the TANF (Temporary Assistance for Needy Families) program, the STAR plan is mandatory. If you have not yet selected a primary care physician, the state has selected one for you. You must contact your primary care physician to obtain an out-of-network referral and pre-certification for X-rays and MRI/CT scans.
Patients receiving benefits under the SSI program may choose to participate in the STAR program or traditional Medicaid.
HMO/PPO/POS
Generally, our physicians do not participate in these types of plans. Please contact your network to obtain an out-of-network authorization. If you provide an authorization number at the time of your visit, we will collect only your co-pay or co-insurance amount and file your claim. You also need to check with your insurance regarding a need for pre-certification for X-rays or MRI/CT scans.
